Suppose you’re an employee and have previously included family members — such as your spouse or parents — as co-insured individuals without their income. In that case, it’s important to know that this benefit will no longer apply automatically. Beginning in September, they will lose their insured status in the public health system unless you, as an employee, take the necessary steps to maintain their coverage.

What Changes Does Law 141/2025 Bring to the Healthcare System?

Law no. 141/2025, published in the Official Gazette on July 25, 2025, is part of the new fiscal measures package and introduces major changes regarding health insurance. Specifically, it eliminates the possibility of being co-insured without paying the health insurance contribution (CASS).

More precisely, the following categories are no longer exempt from paying CASS: Spouse or parents without their own income who are supported by an insured individual; Individuals on adoption or parental leave; Monastic personnel; Individuals receiving unemployment benefits, among others.

In practical terms, simply being an insured person (e.g., an employee) no longer automatically grants insured status to your spouse or parents who have no income of their own.
These individuals will only remain insured until September 1, 2025.

Starting August 1, 2025, those who wish to maintain health insurance coverage for a spouse or parent without income can do so by submitting the Single Tax Declaration (Form 212) and paying the annual health insurance contribution. This contribution is set at 10% of six gross minimum wages, which amounts to 2,430 lei for 2025. Payment can be made in two installments, as outlined by law.

 

How Can You Maintain Health Coverage for Family Members?

Starting from August 1, 2025, if you want your parents or partner without income to remain medically insured, you must complete Form 212 and pay the health insurance contribution.
The contribution is calculated as:
2,430 lei per year (10% of six gross minimum wages for 2025)

Payment Schedule:

  • 25% (607.5 lei) upon submitting the declaration;
  • 75% (1,822.5 lei) by May 25, 2026.

Who Can Be Insured in the Public Healthcare System (Paid Option)?

You can include the following individuals in the public healthcare system by paying the contribution:
✔️ Your spouse with no personal income
✔️ Your parents, if they are dependent on you and have no income
